Provide complete information
Provide accurate and complete information concerning present complaints, past medical history, and other matters related to health.

Follow the treatment plan
Follow the treatment plan established by your physician, including the instructions of nurses and other health-care personnel.
Keep appointments
Keep appointments and notify the office or physician when unable to keep a scheduled appointment.
Take responsibility for decisions
Take responsibility for actions and consequences if you refuse treatment or do not follow your physician's orders.
Notify for emergencies
Promptly notify your physician and/or health plan of any situation requiring emergency care or admission.
Laboratory Testing
Clínica Médica Familiar has a fully equipped on-site laboratory which is CLIA and COLA certified. However, depending on your health plan, testing may not be covered by your insurance and you will be directed to your health plan's contracted laboratory. We do not base treatment recommendations on coverage — we'll work with you to find the right path.
NO COST, FREE Services for eligible patients
Clínica Médica Familiar offers free services to those who qualify. Programs include Child Health (CHDP), Presumptive Eligibility, Perinatal Services, Family Planning (PACT), Healthy Families, and Cancer Detection. To facilitate your initial welcome visit, please download and complete the specific forms outlined by program in English and Spanish — or complete them at the clinic on arrival.
What to Bring to an Appointment
Please bring (1) your current valid insurance card, (2) a photo I.D., and (3) medical records, relevant and recent labs, and imaging reports. Intake forms are available in English and Spanish for each NO-COST program.
