A no-cost, FREE program for eligible children — physical exams, immunizations, vision and hearing screening, dental and nutritional assessments, and more.

A special interest in children's health

At Clínica Médica Familiar, we have a special interest in the health and well-being of our community's children. The clinic participates in a NO-COST, FREE Child Health and Disability Prevention Program (CHDP) for eligible clients.

About CHDP

The CHDP program oversees the screening and follow-up components of the federally mandated Early and Periodic Screening, Diagnosis, and Treatment program for Medi-Cal eligible children and youth. In July 2003, the CHDP program began using the "CHDP Gateway," an automated pre-enrollment process for non Medi-Cal, uninsured children. The program provides complete health assessments for the early detection and prevention of disease and disabilities for low-income children and youth.

What a CHDP health assessment includes

Health history

Comprehensive review of medical history and family history.

Physical examination

Complete physical exam by a CMF physician.

Developmental assessment

Age-appropriate developmental milestone evaluation.

Nutritional assessment

Diet and growth evaluation, plus age-appropriate nutrition guidance.

Dental assessment

Visual oral and dental check, with referral if treatment is needed.

Vision & hearing tests

Screening to catch issues that can affect learning and development.

Tuberculin testing

TB screening as recommended for age and exposure risk.

Laboratory tests

Including anemia screening and lead screening as appropriate.

Immunizations

Catch-up and routine vaccines per the recommended schedule.

Anticipatory guidance

Health education for parents and youth on healthy living and safety.

Referrals

Referrals to specialists for any needed diagnosis and treatment.

Enrollment forms (birth to 18 years)

Pre-enrollment forms are available in English and Spanish, including the Pre-enrollment Application, Screening List Vaccines, "What Does Your Child Eat?", Pediatrics TB Assessment Questionnaire, and age-appropriate Staying Healthy Assessments for ages 0-6 months through 12-17 years, plus Youth Nutrition (8-21 years). You can download and complete them before your visit, or fill them out at the clinic.
